DD Waiver Case Management: Safe and Structured

The Developmental Disabilities Waiver (“DD Waiver”) program is designed to ensure that individuals with developmental disabilities are able to lead an active life and avoid institutionalization.

Program participants work with a team to craft an individualized service plan and budget designed to achieve their dreams, ensure their health and safety and provide supports in all life areas as needed, including at home, in the community and at work.

Visions Case Management was founded in 1994 specifically to provide high quality case management to DD Waiver participants, and we’ve been doing so ever since. Our experienced Case Managers are ready to help you with all aspects of the program, including maintaining eligibility, organizing and completing required paperwork, setting up a budget that meets your needs, working with state and private agencies, and advocating for you and your rights when challenges arise. After more than 25 years, nobody knows the DD Waiver better than we do, and our Case Managers are ready to work on your behalf.

I have worked in the medical and the social services field for over 20 years. I have a degree in Social Work and graduated from Eastern New Mexico University. I have worked with children and adults in the public schools, state agencies and other private sectors and continue to enjoy working with so many amazing individuals. Working with Visions, I cover the Santa Fe area.

I am native to northern New Mexico and have lived in New Mexico all my live. I currently live in Santa Fe, NM and have been living there for 7 years. Prior to Santa Fe, I lived in other parts of NM. I come from a large family and enjoy many of New Mexico’s wonderful traditions. I love New Mexico’s diversity and beauty and could not think of a better state to live in. I love being outdoors and spending time with my family. In my spare time, I love hiking, fishing and taking pictures of our beautiful state. I enjoy meeting new people and being introduced to different cultures and food, so I take the opportunity to try new things.

I have been with Visions Case Management since June, 2016. I retired from the State of NM on May 31, 2016. During my career with the state, I worked in different capacities as a social worker providing case management and service coordination. My experience ranges from working with families, adults, and children ranging in ages from 0-17 and with the elderly/disabled population. My education and background is in Social Work at a Masters level. I was born in Espanola, NM and grew up in the Chamita area. I moved to Guadalupita, NM (Mora County) in March, 2000 and continue to live in this beautiful, mountain area. My favorite hobbies are reading, scrapbooking, camping and just spending time with family. I have three beautiful children, a son-in-law and a husband whom I have been married to for nearly twenty years. I enjoy being at home during the weekends, but when given the opportunity, I like going out to watch a good movie or taking a drive to visit family. My spirituality also plays a big role in my life and this is what keeps me centered.

I joined the Visions team in May 2018 but have been in the field for about 7 years in different roles prior to this position. Most of my background is in the social services sector. I’ve worked in different roles in the DD waiver and did case management for troubled youth for some time. I was born in California and moved to Santa Fe New Mexico with my family when I was 3, I have been here since then. I have served different regions of New Mexico along my years of experience; in my previous job half my caseload resided in Espanola and in Rio Arriba county. In my current position with Visions I serve in Santa Fe and Albuquerque. I’m extremely passionate about my job and enjoy what I do very much! I have two boys that keep me extremely busy when I’m not working. All the things I enjoy doing outside of work revolve around them. Some of my favorite things to do with my children include road trips, camping, and being involved in their school events. During my free and alone time, I enjoy hiking and love a spa day when I can get away with one!

I have been with Visions Case Management for almost 5 months. Before I became a Case Manager I worked for another agency in the DD waiver field. I have a number of background experience from Direct Care Staff, Quality Assurance, Human Rights Committee Coordinator, House Manager, CCSI Manager with 30 Clients and 6 Staff. I wanted to become a case manager and utilize my skills that I have learned throughout my years of experience in the field. After a Year of asking many questions and getting answers from other Case Managers I finally decided to fix my resume and apply for a Job with a Case management agency. I am so grateful for Visions giving me the opportunity to become a Case Manager and utilize those skills. I enjoy the outdoors and going to our Cabin we have. I enjoy hunting, fishing and spending time and making memories with my husband and grandkids. This job has allowed me to do all the wonderful things that I enjoy, I have met so many new families and have the opportunity to help them in the things they need. With Visions it is a wonderful place to work as the other Case Managers, admin and Supervisors are so helpful and nice. I work hard and try to do the best I can as I am still learning something new everyday.

I have been with Visions since May of 2022. I have a degree in Social Work and a license to practice in NM. My background is in Addictions and Mental Health Counseling. I am originally from Santa Fe, New Mexico but lived in Albuquerque for 8 years and recently moved back to Santa Fe. I work with clients in Santa Fe and Espanola. I have a three and a half year old daughter that I love to spend time with. I also love to read, play video games and color/draw. I enjoy spending time with family and friends and taking my daughter to do fun activities.

I have been with Visions since May of 2023. Before that I was with A New Vision CM for 2 years and 3 months, and before that with Unique Opportunities for 9 years and 5 months. I have lived in Albuquerque for 22 years, and serve mostly in the Metro area. My hobbies are: family/friend’s gatherings, traveling, running, road biking, dancing, meditation, personal development books and podcast.

I have been with Visions since May of 2023, and have been a DDSD Case Manager for 17 years total. Before that I was an Early Interventionist and coordinator for school age assessment for services for 6 years. I was also a DDSD IQR Auditor for 1.5 years. I have a Master's Degree in Family Studies (UNM), am a Certified Mediator (UNM), Certified Paralegal (UNM) and US Army Veteran, Sgt. 1981/1985. I was born and raised in the Barales Community of New Mexico, bilingual, educated in the APS systems. I was stationed/Traveled in Germany/Europe for 2 years, a resident of North Carolina for five years and am currently serving Bernalillo and Valencia County clients. I have traveled the US and Europe extensively. Travel is a passion, as well as experiencing other cultures and foods.

I have been with Visions since 2023. I started in the DD field as a nanny for a Downs Syndrome child and have been in the field 35 years now. I'm originally from CA but have lived in Germany for 7 years and NC for 5 and most recently Glorieta NM 2 years. I service Santa Fe, Samn Miguel, Rio Arriba and Sandoval counties. I love cooking, camping and time with my grandchildren.

I have been working as an independent contractor in the Developmental Disabilities Waiver program for more than three years. I joined the Visions Team as a Case Manager in 2022. I hold a Bachelor of Arts in Multi-Disciplinary studies from the University of Texas at El Paso with areas of concentration in Human Behaviors and Interactions as well as Spanish Literature. This degree granted me flexibility in my interpersonal relations skills. I have been providing services in every region of New Mexico where I am needed but I mainly oversee the Southeastern region serving the Lea, Chaves, and Lincoln counties of New Mexico. I have more than 20 years of experience in the health care industry working with individuals with special needs in the context of a medical office. I administratively led a medical practice that worked with patients with down syndrome and autism. I am a native Spanish speaker, and this allows me to communicate effectively with individuals that may not be fluent in the English language. It is always my goal to go above and beyond in support of my clients. I always advocate for their best interests, and I like taking the time to personally get to know each client and their family, to learn about their goals, and their aspirations. I pride myself in providing excellent customer care to the persons entrusted to me under the DD Waiver program. When I am not working, I love spending time with my beautiful family and our 3 dogs. I also enjoy traveling and spending time with friends watching sunsets, playing outdoor sports, cooking up barbeques, bowling, and playing dominoes.

Nick Marjon

I started my career as an educator and enjoyed teaching creative activities (art, music) to children. We made instruments and learned about primary and secondary colors. Even young 2 and 3 year olds would grasp words like "disappear" with water painting activities outside on a warm sunny day. Working with children was great because they come to class so excited to participate in group activities. It made me seek creative activities that everyone could enjoy and saw how successful they were when other classes/teachers asked to implement them as well. I used my experience in the early childhood educational settings and applied it in the field of working with adults with intellectual disabilities. Over time, I realized there were many similarities between the two fields. It's very important to have patience and can be very rewarding teaching and working with people that have different levels of abilities. I've been in the field of working with adults with developmental disabilities for over 10 years. I started in New Mexico as a direct care staff with an ICF home and worked my way up as Program Manager in Supported Living overseeing several homes in Albuquerque. I then worked as a Qualified Intellectual Disabilities Professional Assistant overseeing 4 ICF homes. I moved to California and worked as a Program Manager for Supported Living in the East Bay area overseeing 9 homes from Oakland, to Berkeley, Pleasanton, and Hayward. I also worked as a Service Coordinator for two years in CA where I assisted consumers with obtaining services. I recently moved back to NM and have been working with Visions for over a year. I enjoy it and I am always learning with this job. No day is the same! I am from Albuquerque, NM. I have lived in Illinois and California. I am glad to be back home. Currently, I am serving Bernalillo, Sandoval, Valencia, and Santa Fe counties. I enjoy spending time with my family when I have free time. I love to travel and learn about other cultural foods, religions, history as well as my own culture. I have a Bachelor's in Art with an emphasis in sculpture and have worked with artists in the ABQ community assisting them with their crafts in leather work, silver, setting stones, turquoise, casting jewelry, and refurbishing jewelry. I enjoy drawing with pastels and charcoal. I've always had a passion for art, but spent the majority of my youth playing the violin. I participated in 16 different orchestras and symphonies before I graduated from high school and performed in Carnegie Hall in New York City my senior year.

I have a Bachelors of Science in Psychology from Old Dominion University in 1994 and a Master's of Science in Education in Counseling from ODU in 1998. I have been working professionally in the mental health field since 1999 I started working with DD clients in 1994 with internship, and then again as a Case Manager since 2003. I have been with Visions since 2006. Originally from Virginia Beach, Virginia, I moved to New Mexico in 2001, and currently live in Ojo Caliente, NM. I serve the Rio Arriba and Los Alamos counties. My hobbies include lots of craft mediums, traveling, and hanging with family.

I am a Native New Mexican and graduate of UNM.
I have over 25 years of experience providing support and advocacy to individuals and family members through a variety of waivers including DD Waiver, Supports Waiver, and Aged and Elderly. I also have experience in Early Intervention and SSA Disability. My heart and home will always in be the Rio Grande Valley. Our blue skies, breathtaking sunrise and sunsets, and smell of roasting Green Chile in the fall is happiness that you can not find anyplace else. In my free time I enjoy cooking, crafting, and taking pictures of our New Mexico Sunrise and Sunsets.

My name is Tracy Reeves and I joined Visions Case Management in 2024.   I have a son who is autistic, so I love to support parents, children and adults to get the services they need to live life to the fullest. Before doing case management I was living in Peru as a missionary and one of my main duties was to help individuals find proper medical care.  My son grew up in Peru, so I became familiar with the medical community as I sought resources for him. When I am not working, you will find me at the gym.  I enjoy being active and am working toward the goal of completing the Camino de Santiago in Spain.
